Presenting Community Health Assessment Data

On the basis of information collected during Step 2: Conducting a Community Health Assessment, both quantitative and qualitative data is shared among community members to not only help raise awareness but also to start the process of determining priorities. Findings from the community health assessment should help inform the prioritization process by focusing on:

  • Who is most affected by current policies, practices, and environmental factors related to nutrition and physical activity.
  • What factors contribute to unhealthy nutrition and the lack of physical activity.
  • What strategies, perceptions, and opinions the community has shared.
