Healthy Eating Active Living (H.E.A.L.) Assessment Tool
The Healthy Eating and Active Living (HEAL) Assessment is designed as a self-assessment and planning guide to help you define or identify your community baseline in terms of policies, practices, and environmental factors. Taking stock of the resources, assets, deficits and needs within the community can help illustrate the current local picture of obesity in terms of knowledge, attitudes, beliefs and behaviors related to physical activity and nutrition. The purpose of this assessment is three-fold:
- To gather information about nutrition and physical activity environments, practices, and policies within a variety of settings of a particular geographic boundary;
- To enable community members to address and explore opportunities for partnerships and collaboration; and
- To facilitate decision making of community interventions
