Provided as supplemental material to assist communities in conducting a community health assessment is the:

Healthy Eating and Active Living (HEAL) Assessment Tool
A self-assessment and planning guide to help define or identify the community baseline in terms of policies, practices, and environmental factors.

Community Opinion Sample Survey
A survey of the community-at-large which provides viewpoints about healthy eating and active living awareness, needs, and perceived barriers. Community opinion surveys allow data collection for large numbers of respondents on a variety of health behaviors and perceptions. Community settings, worksites, parks and other recreation sites, as well as door-to-door or telephone data collection are useful for obtaining data.

Community Focus Group Discussions
A survey of specific focus groups which provides viewpoints about healthy eating and active living awareness, needs, and perceived barriers. Focus groups are selectively held with target populations to gain better insight into their perceptions of the issues of concern and how to address them. Sessions usually involve a facilitator, note taker, and 8-10 participants.
